A Clinical Study on Acute Generalized Peritonitis

Abstract
This report is a clinical analysis of 520 patients with acute generalized peritonitis, who underwent the abdominal operations at this hospital during the period from June, 1979 to February, 1983.
The results obtained are as follows:
1) The main causes of acute generalized peritonitis were traumatic peritonitis (34.6%), perforated peptic ulcer(23.5%), perforated appendicitis (21.7%), typhoid peritonitis(4.4%), bile peritonitis(3. 1%) and miscellaneous causes (12.7%).
2) The majority of patients with tuberculous peritonitis, gynecologic disease, duodenal ulcer perforation and traumatic peritonitis were in the 3rd to 4th decade; with perforated appendicitis in the 2nd to 3 rd decade and 7th decade; with bile peritonitis in the 7 th deca-de or over with typhoid peritonitis in the 4th to 5th decade; with rupture of hepatoma in the 5th decade or over.
3) The incidence of leukocytosis was over 70%. in, traumatic peritonitis, peptic ulcer perfo-ration and perforated appendicitis, 43% in bile peritonitis and 40% in typhoid peritonitis.
4) The incidence of subphrenic free air shadow in, the plain X-ray was 67.2% in peptic ulcer perforation, 43.5% in typhoid peritonitis and 6.9% in traumatic hollow viscus perfor-ation.
5) The diagnostic accuracy rate of paracentesis was 87% in traumatic peritonitis and 79.3% in non-traumatic peritonitis.
6) Routine cultures of aerobic organisms from peritoneal fluid revealed E. coli, Citrobacter, Klebsiella, Enterobacter and Staphylococci in order of frequency.
7) The most frequent complication was wound infection (5. 8%), followed by pulmonary complication (4.4%) and sepsis (2.9%) in that order.
8) The overall mortality was 2.9%.
9) The most frequent cause of death was sepsis, followed by shock lung, irreversible hemorrhagic shock, cerebral contusion and hepatic failure in that order.
